The Out of Home Care – Referral Placement List provides a statistical report outlining referrals and placements for the specified date range.

The Out of Home Care – Referral Placement List provides a report for the specified period consisting of 2 tables:

Referrals and Children

    • Child
    • Aboriginal / Torres Strait
    • CALD
    • Placement Type Requested
    • Referral Date
  • Siblings requested to be placed together

Placements and Children

    • Carer Household
    • Placement Type
    • Placement Commenced
    • Child
    • Sibling Group
    • Siblings Placed Together
    • Aboriginal / Torres Strait
  • CALD

Instructions

Step 1. Log into your CRM database (refer to Logging On for further information).

Step 2.  Select Report button from the Top Menu

Step 3. From the Report drop-down menu, select  Out of Home Care – Referral Placement List.

The Out of Home Care – Referral Placement List report has the capacity to be limited by start and end dates. These fields are located under Parameters; complete them according to your requirements.

Step 4. Click Run Report

Once the report has completed processing, click the ‘Click Here to Download’  link

Download Report

The Report will display in your browser, or open with your PDF viewer, depending on your computer’s settings.

Out of Home Care – Referral Placement List Report is displayed, including the following information:

  • Referrals received within the reporting period.
  • Placements commenced within the reporting period.
