DSS SCORE Assessment for a Group

DSS provides 1 group assessment domain for community case work:

  • Community domain

Group assessment only varies from individual assessment by applying to a group of clients instead of individuals.

As such users are not required to click on a session attendance record and instead use the ‘Group Assessment’ panel to create pre-session and post-session assessments.

Instructions

Step 1.  Log into your CRM database (refer to Logging On for further information).

Step 2.  Locate the session you wish to assess.

Note:
This may exist under any Community Education or any Module that is for Group Work.
Session Record

Step 3. Select the completed session.

Tip:
This would be a session that has had a client interaction i.e. Attended, Phone Call, email etc

Step 4.  Scroll to the Group Assessments Sub-panel

Step 5. Select Create a Group Assessment

Step 6. Select the 3 required fields of information

a. Assessment Template
b. Assessment Type
C. Assessed by

Tip:
The screenshots below are an example only.
Select the appropriate options for the Assessment you want to do.

c. Assessed By

Step 7.  Click Save

Step 9. The Assessment questions section are displayed

Step 9. Complete the questions as required

Step 10. Click Save

The Assessment record is saved

Step 11. Repeat steps 5 – 10 for any additional assessment types for the group.

These may include:

  • Initial
  • Subsequent
Tip: Ensure there is only one assessment of each type. 
If multiple assessments of the same type are recorded, 
your DEX Portal upload will error.
